Preschool Registration Forms

In order to secure a place in St. Raphael the Archangel Pre-School A for the 2021-2022 academic year, you must submit by February 26, 2021 your enrollment form as well as a non-refundable $100 registration fee per child. You will be informed by March 15th via mail of your child's enrollment status. If the program fills and we are not able to accommodate your child, you will be notified by letter, the registration fee will be refunded to you, and you will be placed on a waiting list.

Hierarchy of Acceptance- All forms turned in by February 26th will be given priority based on these conditions 1)Parishioners of St. Raphael the Archangel 2)Returning families currently enrolled at St. Raphael the Archangel School or Pre-K 3)Full time or full days students 4) Availability of part-time spots. After February 26, enrollment will be handled on a first-come, first-serve basis.

Transferring Students Forms

Forms that need to be completed for enrollment packet to be complete:

• Admissions Form
• Financial Letter of Intent
• Emergency Contact Form
• Non-refundable Enrollment Fee submitted to the school office
• FACTS Automatic Withdrawal Online (If you have not enrolled already)
• Today and Tomorrow Education Fund (TTEF.org)
• Student Assessment Form (for students enrolling in Kindergarten)
• Request for Information from Current School
• A copy of your child's Baptisimal Certificate if the Baptism was not at St. Raphael's
• A copy of your child's Birth Certificate
• If you are not Catholic, a letter of recomendation from you minister. If at present you do not have a religious affiliation, please indicate that in a note to us.
